PAT Ministry Report Form B

This form is for members of the Pastor's Advisory Team to complete on behalf of the Ministry Heads on their team. Please include any programs or events and numbers of participants or other useful data. 

(In the example, Parish Events completes a form including their 3 separate ministries: Special Events, Asian American Ministry, and BINGO. The # of items reporting field will open the form up for 3 entries, and each ministry gets it's own section of the form.) SEE SAMPLE FORM
