PAT Ministry Report Form

This form is for members of the Pastor's Advisory Team to complete on behalf of the Ministry Heads on their team. Please include any programs or events and numbers of participants or other useful data. Use one field below for each additional event. 

(For example, Community Building should complete a form for Respect Life, that form should have a field for the amounts of diapers donated in the most recent drive, anther entry for the amount of people who participated in prayer outside a clinic, etc.) SEE SAMPLE FORM
