Ministry Heads Request Forms

Do you have a special event or project for your event?
Please complete the following form SIX WEEKS prior to the event. Don't forget, it's always a good idea to reserve your dates as far out as possible through Anita Tooke in the Parish Office at 904-282-0439 or  via email at anita@stlukesparish.org.

The Event Form should be completed with the Ministry Head Procedures at hand in order to know deadline schedules, etc.

All print, electronic, and social media is managed by the St. Luke communications office. Requests for your ministry's information to be published do not guarantee that your content will be printed/posted. All content is subject to schedule availability and approval from the pastor.
